Skilled in Medical Care
These services are provided by our Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), focusing on medical treatments and health monitoring.
Skilled Nursing: Our nurses manage complex health needs, including post-surgical observation, administering IV therapy and injections, and clinical assessments. We focus on educating you and your family to ensure confidence and competence in managing your health.
Wound Care Management: We provide expert care for chronic and complex wounds, including dressing changes, monitoring for infection, and collaborating with your physician to promote optimal healing.
Medication Management: Oversight of complex medication regimens, ensuring medications are taken correctly and on schedule, including administering and educating on new prescriptions.
Chronic Disease Management: Specialized teaching and monitoring for conditions such as diabetes, Congestive Heart Failure (CHF), and COPD, focusing on symptom control and preventing hospital readmission.
